Angizem CD 120 is a prescription medicine used to treat mild to moderate high blood pressure and effort-associated angina (chest pain). This medicine should normally be given to patients whose treatment with other antihypertensive medicines has been ineffective or has been related to unwanted adverse effects. The medicine contains the active ingredient Diltiazem, which is an oral calcium blocker used in many clinical scenarios as an antiarrhythmic, antihypertensive, and anti-anginal.
Angizem CD 120mg helps treat long-term conditions of high blood pressure and chest pain but does not cure them. Therefore, the medicine must be given every day. It may be used alone or in combination with antihypertensive medicine to lower blood pressure. Diltiazem helps to reduce blood pressure by causing relaxation and widening of blood vessels. It also helps the heart pump more effectively. When it comes to treating angina, this medicine reduces the amount of oxygen that your heart muscle requires.
Angizem CD 120mg is contraindicated in the following conditions/situations:
- Sick sinus syndrome except in the presence of an implanted pacemaker
- Known hypersensitivity to Diltiazem
- A heart blockage
- Severe hypotension (lower blood pressure than the normal range)
- Myocardial infarction
- Concomitant use of dantrolene infusion
- Pregnant women
- Concomitant use of Ivabradine
- Concomitant use of lomitapide.
Before you use Diltiazem, talk to your healthcare specialist if you have low blood pressure, heart problems, liver disease, kidney problems, heart disorders, a recent heart attack, diabetes, or are 65 years or older. Alcohol consumption may cause a significant reduction in blood pressure and dizziness, especially when you stand from a lying or sitting position. This may usually occur after taking the initial dose and when the dose is increased. Let your healthcare specialist know if you develop lightheadedness, decreased blood pressure, or increased heart rate. Consumption of grapefruit/grapefruit juice too often while taking Diltiazem may cause oedema, chest pain, irregular heartbeat, and unexplained weight gain. Let your healthcare professional know if this happens to you. It is recommended that you avoid grapefruit juice if this happens.
Like other medications, interactions with other drugs are possible. Tell your healthcare specialist about all medications you take, including prescription medications, mineral supplements, vitamins or alternative medicines.
Here are some medications that may interact with Diltiazem. These include:
- Medications to control the immune system, such as cyclosporin
- Heart medicines such as amiodarone, digitalis, quinidine, verapamil, nifedipine, digoxin, quinidine, flecainide, propafenone
- Medicines to control seizures, including phenobarbital, carbamazepine, phenytoin
- Anticoagulant such as warfarin
- Medicines that dilate blood vessels, such as short and long-acting nitrates
- Anaesthetics
- Sleeping pills such as triazolam and midazolam
- Imipramine and Lithium for some types of mental illness
- Theophylline is used for breathing problems
- Cholesterol-lowering medicines (statins)
- Dantrolene for severe muscle spasms or severe fever
- Medications to control stomach ulcers, such as cimetidine, will increase the effect of Angizem
- Terfenadine or ranitidine for allergies
- Certain antibiotics should not be used with Diltiazem, such as rifampicin erythromycin.
- Multivitamins (calcium-containing products)
- Corticosteroids, methylprednisolone, to treat inflammation
- Dantrolene for severe muscle spasms or severe fever
The most common side effects of Angizem CD 120 mg include nausea, constipation, indigestion, swollen ankles and dizziness. Contact your healthcare provider if any of these worsen.
For hypertension: When used as a single medication, the usual starting doses are 180 to 140 mg once a day, although some patients may respond to a 120 mg dose once a day. In general, the maximum antihypertensive effect is usually observed after around 2 to 4 weeks of treatment; therefore, the dose adjustment should be scheduled accordingly. The daily dose should not exceed 360 mg. The dosage of Diltiazem or concomitant hypertensive medicines may require to be adjusted when adding one to the other.
For angina: The dosage from the treatment of angina should be adjusted to each patient’s requirement. The usual starting dose is 120 mg to 180 mg once a day. Individual patients may respond to higher doses of up to 120 to 180 mg once a day. When required, the dose adjustment should be carried out over a 1 to 2-week period. There is limited experience with doses exceeding 360 mg. However, the incidence of side effects increases as the dose increases, with heart blockage, sinus bradycardia, and dizziness bearing the strongest association with the dose. Therefore, it is not recommended to use a dose greater than 360 mg.
Method of Administration: The capsules should not be opened or chewed. The tablet must be taken exactly as prescribed by your healthcare provider. Avoid missing doses or taking extra doses unless your healthcare specialist advises you.
