Zeptol controlled release 400 mg is an anticonvulsant medicine prescribed to treat trigeminal neuralgia, all forms of partial epilepsy, generalized tonic-clonic seizures, and simple, complex partial seizures. It is also approved for the treatment of symptoms of bipolar 1 disorder, which involves episodes of depression and/or mania.
Symptoms of epilepsy
- Temporary confusion
- Loss of consciousness
- Muscle spasms
- Sensory disturbances
- Uncontrolled jerking movements
Symptoms of depression include:
- Depressed mood
- Feeling worthless, hopeless, helpless
- Sleep more or less than usual
- Loss of interest or pleasure in daily activities
- Trouble concentrating, low in energy, or thoughts of death
- Nervous energy
- Feeling like you are moving in slow motion
Symptoms of mania include:
- Feeling irritable
- Having increased self-esteem
- Feeling the need to continue to talk
- Feeling distracted
- Feeling like your thoughts are too quick
- Getting engaged in activities that are risky or could have bad results
The medicine contains the active ingredient Carbamazepine, an anticonvulsant medicine that functions by stabilizing the electrical activity in the brain and nerves. This medicine stops electrical signals from building up in the nerve cells in the brain.
Do not take Zeptol CR if you:
- Are allergic to Carbamazepine or any other ingredients of the medicine
- Have a history of bone marrow depression
- Take nefazodone
- Are allergic to tricyclic antidepressants.
- Have taken monoamine oxidase inhibitor in the last two weeks
Furthermore, before you take this anti-epileptic agent, be sure to inform your healthcare provider if you:
- Have had heart, blood/liver/kidney problems
- Have had increased pressure in the eyes
- Eat grapefruit or drink grapefruit juice
- Have any other medical conditions
- Use contraceptive measures. Zeptol CR may make a contraceptive pill less effective. Inform your healthcare specialist straight away if you become pregnant during the treatment. You and your healthcare professional should decide if you should take this medicine during pregnancy.
- Are breastfeeding or plan to breastfeed. The medicine passes into breast milk. You and your healthcare specialist should determine whether you should take this medicine or breastfeed. Generally, it is not recommended to do both.
Here are the medicines that may increase/decrease the levels and effects of Carbamazepine:
- Certain antibiotics such as ciprofloxacin, clarithromycin
- The mood stabilizer and antiseizure medicine valproic acid/Divalproex
- Medicines for heartburn or reflux, including cimetidine
- Certain heart medicines, including diltiazem or verapamil
- Antifungal medicines such as voriconazole, fluconazole, and ketoconazole
- Certain medicines for enhancing mood or sleep, such as trazodone or nefazodone
- Phenytoin, primidone, phenobarbital
- Rifampin
- Oral contraceptives
- Anti-rejection medications used in organ transplants, like cyclosporine and tacrolimus
- Certain cholesterol medicines, such as atorvastatin and simvastatin
- Certain medicines for treating psychiatric disorders, such as aripiprazole, lurasidone, escitalopram, nefazodone, trazodone,
- The blood thinner medicine
- Certain cholesterol medicines like atorvastatin and simvastatin
The most common side effects associated with the use of Zeptol CR 400 mg include drowsiness, dizziness, nausea, vomiting, and problems with walking and coordination.
Zeptol CR 400 mg may also cause serious adverse effects, which include:
- Liver problems- Symptoms are dark urine, nausea, easy bruising, loss of appetite, jaundice (yellowing of your skin or the whites of your eyes), vomiting, abdomen pain
- Irregular heartbeat- Symptoms are breathlessness, feeling lightheaded, fast/slow or pounding heartbeat

Zeptol controlled-release tablet needs to be taken with food. Make sure the tablet should not be crushed, chewed or broken. Do not discontinue treatment without first discussing it with your healthcare professional. Stopping the medicine suddenly in a patient who has epilepsy may contribute to seizure attacks that will not stop.
