Dicorate ER 500 mg is used to treat acute manic or mixed episodes accompanied by bipolar disorders with or without psychotic features. It is prescribed as monotherapy and in combination treatment for adult patients and pediatric patients (aged 10 years or older) accompanied by complex partial seizures that occur alone or along with other seizure types. It also treats simple and complex seizures and is used to prevent migraines. 

Dicorate ER 500mg contains Divalproex sodium, an anticonvulsant as the main active ingredient that works in the brain tissue to stop seizures. The medicine is believed to increase brain chemicals called GABA that help slow down electrical brain activity.

 

Precautions with Dicorate ER 500 mg?

Dicorate ER 500 mg is contraindicated in the following conditions/situations:

  • Hypersensitivity to Divalproex sodium or valproic acid
  • Patients with a family history of severe hepatitis
  • Acute and chronic hepatitis
  • Genetic liver issues caused by a mitochondrial disorder (e.g. Alpers-Huttenlocher syndrome)
  • Using medicines for the prevention of migraine headaches.
  • Pregnancy
  • Patients with known urea cycle disorders
  • Patients with known porphyria

Dicorate extended-release tablets may harm your unborn baby. Therefore, women of reproductive potential need to use effective methods of birth control. It is recommended to use effective birth control such as the pill and a condom. Women should use birth control for at least a month before taking Dicorate ER, during the entire treatment, and after stopping the treatment. Consulting a pediatrician is important for the use of this medicine in children. For older patients, it is recommended to reduce the starting dose, increase the dosage more slowly, and closely monitor nutritional intake.

Dicorate ER 500 mg should be withdrawn gradually to minimize the risk of seizures or increased seizure frequency. Combining Divalproex with certain medications may affect the levels of antiepileptic medicine. These include aspirin, rifampicin, antibiotics (meropenem, ertapenem), anticonvulsant drugs (phenytoin, carbamazepine, and phenobarbital), antidepressants (amitriptyline and nortriptyline), Diazepam, oxcarbazepine, and Olanzapine. 

The common side effects of Divalproex Dicorate ER 500 mg include sleepiness, double vision, vomiting, nausea, weakness, dizziness, increased appetite, hair loss, weight gain, problems with walking or coordination, stomach pain, blurry vision, and tremors. 

Divalproex may cause serious side effects as well. These include:

  • Low blood count: Appearance of red or purple spots on your skin, bleeding, and bruising from your nose, mouth, and teeth. 
  • Hypothermia (Low body temperature): A drop in body temperature to less than 95 degrees Fahrenheit, accompanied by confusion and coma.
  • Allergic reactions: Fever, hives, skin rash, swelling of your lymph nodes, blistering and peeling of your skin, swelling of your eyes, face, lips, throat, tongue, or throat, trouble swallowing or breathing.
  • Drowsiness or sleepiness in elderly patients: Extreme drowsiness may make you more likely to eat or drink less than usual. It is essential to inform your healthcare expert if you cannot eat or drink like you normally do. Your healthcare specialist may start treatment with the lowest possible dose.

How to use Divalproex sodium?

Patients receiving combined antiepileptic treatment require careful monitoring when another medicine is initiated, discontinued, or when the dose is altered. Bipolar disorder requires long-term therapy, even when you feel better. Sudden discontinuation of Dicorate ER 500 mg in patients can be life-threatening.

Changes in dosage or the addition or discontinuation of concomitant medications require close monitoring.  

Patients who experience gastrointestinal irritation may benefit from taking the medication with food or a progressive increase from the initial low level. Dicorate extended-release tablets should be swallowed without chewing. Taking this medicine with food should cause no clinical health problems in the management of patients with epilepsy.
