Herpex 5% Cream is an antiviral cream that can treat cold sores caused by herpes virus infection. What medications interact with Herpex cream?

Experts have not identified any major interactions resulting from topical or systemic administration of other drugs concomitantly with Herpex cream. In some rare cases, its application decreases urinary excretion and increases half-life. To some extent, a risk of renal impairment increases when using with other nephrotoxic drugs. This cream is contraindicated in patients with a pre-existing hypersensitivity to valaciclovir, aciclovir, or any component of the formulation.

It treats localised skin HSV but is not suitable for oral, genital mucosal, or systemic herpes.

How should Herpex 5% Cream be applied?

Apply Herpex 5% Cream to the cold sore lesion five times daily (approximately every 4 hours during waking hours). Begin application at the very first sign of a cold sore — the prodromal tingling or itching sensation before any visible lesion appears. Apply a thin layer to the lesion and approximately 1cm surrounding normal skin. Use a clean fingertip (wash hands immediately after) or cotton swab to avoid spreading virus. Continue treatment for 5 days. Avoid contact with eyes and mucous membranes. If the lesion has not healed after 10 days of treatment, consult a physician.

How quickly does Herpex 5% Cream work on a cold sore?

When applied during the prodromal phase (before the blister appears), Herpex Cream can prevent full blister formation in some patients or significantly reduce blister size. When applied to an already-formed blister, it reduces healing time by approximately 1–2 days (from the typical 7–10 days to 5–8 days) and reduces the duration of pain and viral shedding. Clinical benefit is greatest with early application — studies show the difference in healing between early-treated and untreated cold sores is most pronounced when treatment starts within the first 24–48 hours of symptom onset.

Is Herpex 5% Cream safe to use for children with cold sores?

Herpex 5% Cream can be used in children (typically age 2 and over) for cold sores on the lips and face. Apply the same way as for adults — five times daily for 5 days. Avoid getting the cream near the eyes or in the mouth. In young children, adult supervision of application is essential. For children with frequent cold sore outbreaks, oral acyclovir suspension may be more practical than topical cream. Neonates and infants with suspected HSV infection require immediate medical assessment and likely IV acyclovir — do not use topical cream for neonatal herpes.

The cream base (polyethylene glycol) may interfere with latex condoms and diaphragms if applied to genital skin — use other contraception if topical cream is applied near genital areas.
