Telma H 40/12.50 mg is basically a combination of two medications, Telmisartan and Hydrochlorothiazide, in a single tablet. Both medications work to control high blood pressure. Telmisartan is categorised under the class of medications known as angiotensin II receptor blockers. Angiotensin II is a component released in your body which contributes to the narrowing of the blood vessels, thus increasing blood pressure levels. Telmisartan works by blocking the activity of angiotensin II, which causes the relaxation of blood vessels, thus lowering blood pressure. Hydrochlorothiazide is a member of the thiazide diuretics group of medicines. It increases urine output, ultimately lowering blood pressure.
Untreated, uncontrolled blood pressure can damage blood vessels in several organs, which could cause stroke, heart attack, blindness, or heart or kidney failure. High blood pressure shows no symptoms before damage occurs. Thus, it is important to measure blood pressure regularly to check whether it is within the normal range. Telma H effectively treats uncontrolled blood pressure in adults whose blood pressure is not sufficiently controlled when Telmisartan is used alone.
Do not take Telma H if you are allergic to Telmisartan, Hydrochlorothiazide, or any other ingredients of the medicine.
Avoid taking this medicine if any of the following applies to you:
- Are more than three months pregnant, or have used any other sulfonamide-derived medicines.
- Have severe liver problems such as biliary obstruction, cholestasis, or any other liver disease.
- Low levels of potassium or high calcium levels in your bloodstream do not get better with treatment.
- Have impaired kidney function or diabetes, and you are treated with a blood pressure medicine containing aliskiren.
The use of Telma H in children and adolescents up to 18 years of age is not recommended.
Inform your healthcare professional if you are taking any other medicines. You may be advised to change the dose of these other medicines or take other precautions to avoid drug interactions. In some cases, you may have to discontinue using one of the medicines. This applies especially to the medicines given below:
- Medicines associated with low blood potassium, such as other diuretics, laxatives, corticosteroids, and amphotericin. Penicillin G sodium, carbenoxolone, and salicylic acid and derivatives.
- Medicines that may increase the levels of potassium in the bloodstream include potassium supplements, potassium-sparing diuretics, ACE inhibitors, cyclosporin, and other medicinal products such as heparin sodium.
- Medicines for the treatment of diabetes (insulin or metformin)
- Colestipol and cholestyramine for lowering blood fat levels
- Noradrenaline to increase blood pressure
- Calcium D or vitamin D supplements
- Muscle-relaxing medicines such as tubocurarine
- Amantadine to treat Parkinson’s disease and to treat or prevent certain illnesses caused by viruses.
- Anticholinergic medicines to treat a variety of disorders such as urinary bladder spasms, motion sickness, gastrointestinal cramps, asthma, and muscular spasms
- Other medicines to treat high blood pressure, painkillers, corticosteroids, and medicines to treat cancer, arthritis, or gout
- If you are taking Aliskiren or ACE inhibitors, avoid using this medication
- Digoxin
These are not all the possible side effects you may develop when taking Telma H; if you develop any adverse effects not mentioned here, inform your healthcare provider. Side effects may include:
- Headache, dizziness, anxiety, pins and needles in your fingers.
- Rash, skin eruptions, red patches on the skin
- Dry mouth
- Upper respiratory tract infection
- Muscle cramps, weakness, back or leg pain, restlessness, joint pain, muscle spasms
- Insomnia, drowsiness, fatigue
- Reduced libido
- Acute respiratory distress (symptoms include weakness, confusion, shortness of breath, and fever.
If you develop breathing or lung problems (inflammation of fluids in the lungs) after taking Telma H, seek immediate medical help.
Telma-H is not for initial treatment. You must first be stabilised on the single components of Telma H. Take it exactly as prescribed by the healthcare specialist. It is always recommended to take the dose at the same time daily, preferably in the morning. The medicine can be taken before or after consuming food. The tablet is for once daily administration and should be swallowed with a glass of water. If the medicine causes an upset stomach, prefer to take it with food or milk.
